We are proud to feature
a paperless office with a modern Electronic Medical Record
system. Our new electronic portal will allow you to check in and
fill out forms electronically, communicate with our office, perform
on-line scheduling requests, view labs and X-Ray reports and do your
medication refill requests online. See eMDs

For now, select forms can be filled out
electronically, in the comfort of your own home. Form
information is not saved on the web site and is either sent electronically
or you can fill it out and mail it to us or FAX it to us. We will
have it processed before you arrive, as long as we get it at least
48 hours before your appointement.
The more complete
your information, the better able we are to help you quickly and
completely. You can fill out the form online or print and FAX it to
us.
